What is an Video Release Form?

A video release form permits you to use someone’s or something’s likeness, voice, or image in your video. Authorization is especially essential if subjects are directly or indirectly identifiable in your content. It is best to obtain consent early, often before shooting the video, to avoid spending time on work that you cannot publish.

The release form must contain the person’s name and contact information and a declaration that they have authorized you to use the video commercially or otherwise. Outline authorized uses and how long the release applies, and make provisions for consent withdrawal, preferably up to a specified number of days before you publish.

If you plan to use the footage for multiple purposes, you can have the models grant permission for flexible future use. Authorization for flexibility of use is necessary for content creators who anticipate using the content in ways that exceed their initial plans. A parent or guardian must sign a video release form for minors because they are too young to consent to it. A comprehensive video release form typically includes permission to feature the subject’s voice, so you do not need a separate audio recording release form. Our video release template should take you minutes to customize and complete.
